Live Scan Information

Please review California Code of Regulations Title 22 Section 100214.3 on Denial or Revocation of a Certificate prior to applying. 

Please be aware EMTs are required to submit a Live Scan Form to the El Dorado County EMS Agency. This form can be found below if not downloaded from the portal. 

Directions for Completion of Fingerprint Background Check

  1. Complete a “Request for LiveScan Service” form (Below)
    • Please make sure the form is filled out correctly.  DOJ may reject it, requiring the background check to be repeated, including additional fees.
    • The background check process must be completed for each Agency that requires one. Information obtained in a DOJ/FBI Background Check cannot be shared between Agencies.
  2. Please have the LiveScan process completed within thirty (30) days prior to the date you plan on applying for certification or recertification - expired one year or longer. Live scan not needed for renewals or within one year of expiration. Contact a LiveScan Agency from the attached list and make an appointment, if necessary.   3. You will need to take a photo I.D. with you to the LiveScan Agency.
  4. You will need 3 copies of the LiveScan form for distribution as follows:
    • LiveScan Operator
    • El Dorado County EMS Agency
    • Your copy
  5. The copy you submit online to the El Dorado County EMS Agency when completing your application and the copy you keep for yourself can be used to track what happens if your LiveScan fails to be processed.

Certification will be issued after review and approval of the application and associated documentation, results of the background check and receipt of the EMR/EMT certification or recertification fee.
