Live Scan

Important!
The Live Scan Application is in Adobe PDF format and must be completed with the free Adobe Reader application to assure the form is properly filled out. Third party PDF viewers like Google's Chrome PDF Viewer and Apple's Safari PDF Viewer may not work properly and using them may lead to an invalid form being submitted.

  1. Complete the Live Scan Application(PDF, 164KB). Some fields have been pre-filled with information specific to the El Dorado County EMS Agency. Do not clear, alter or delete this information. Print out three (3) copies of the completed application.
  2. Take all three copies of the completed Live Scan Application to a Live Scan service provider in your area. Live Scan terminals where you can be fingerprinted are located in sheriffs’ offices and police departments throughout the state, as well as public applicant Live Scan sites. A statewide list of Live Scan terminal locations can also be found at the DOJ Live Scan website.
  3. It is important to note that the El Dorado County EMS Agency Originating Agency Identifier (ORI) Number must be listed on the Live Scan application. This information has been pre-filled into the Live Scan application linked below on this EMS website. If you have not altered or deleted the pre-filled information, there should be no processing delays.
    Live Scan applications processed and DOJ background checks completed without the County of El Dorado EMS Agency’s originating agency identification (ORI) number are invalid for purposes of fulfilling the requirements for EMT certification.
  4. When you decide where to go for fingerprinting, call ahead, even if they do not require appointments, to make sure you have all the information needed for the particular Live Scan site.
  5. Take a photo I.D. with you to the Live Scan agency.
  6. The EMT card can only be issued after the El Dorado County EMS Agency receives the Live Scan results and all other requirements for certification are met.
  7. Live Scans completed more than 90 days in advance of submitting the EMT Certification Application will have to be repeated at the applicants' cost.
  8. Government fees are required and will be collected at the time of the Live Scan. Current fees are: State (DOJ $32) and Federal (FBI $17) for criminal history record checks. Additional fees (Rolling Fees) may also be required (e.g., license or certification fees). Rolling fees can vary at each location.
