California Children's Services Transition Youth

What is transition and why is it important?
California Children's Services participation ends when an individual turns 21. Preparing early can help ensure a smooth transition. It is important to start these conversations with your child as early as the age of 14. Young adults with complex health care needs will need to prepare for navigating the adult health care system. It is important that you adults are well informed and have all the resources they need to make this transition. Care Management Counselors help youth by offering transition planning, making service referrals, connecting clients to enhanced care management, and linking them to community programs and resources.
Here are some important medical questions you should be prepared to answer.
- Does my pediatric doctor see patients over the age of 18?
- Have you spoken with your pediatric care provider about referring you to an adult care provider and specialist?
- Do you understand your medical condition, treatment plan, and medications?
- What medication, equipment, or supplies will you need, and do you know where to get them?
- How will my future plans for independent living, college, and employment be affected by complex health care needs?
